How Do I Become a New Patient?
Transitioning medical care can be difficult, but we are here to help. If you are a military family we know how stressful the frequent transitions in life can be. Here are some things to know before your first appointment.
Coming To Your Appointment
If this is your first visit to our office, completed new patient forms will be required. You will need to present your insurance card, photo ID and copay if required by your insurance company. Parents are encouraged to arrive 15 minutes prior to their appointment time to allow adequate time for all steps to be completed.
What to Expect
We review growth and development, perform a physical examination, and any other anticipatory actions, testing, and immunizations that are indicated for your child’s specific age recommendations.
Documents You Will Need
- Child’s Full Medical History
- Vaccination Records
- Insurance Card(s)
- List of Questions
- Your Driver’s License, or Valid Photo ID
- Family Health History
- School Forms, if needed
- Sports Physical Forms, if needed
Tips and Tricks
- Arrive 15 minutes prior to your appointment time. We understand emergencies can happen but if possible give us 24 hours notice for cancellation.
- Bring any medical records from other providers with you. If not we can complete a medical records request form at your visit.
- Bring any medications (or clear photos on your cellphone) your child is currently taking.
- Ask for refills, school excuses, sports physical forms, or shot records while at your appointment.
After Transferring Paperwork
Once the records are received, we will review them to determine if they are complete or sufficient for us to begin caring for your children. Unfortunately, we cannot treat or provide any care for your children until that time.
Please be aware that under federal (HIPAA) and state laws, your previous medical provider may charge a "reasonable, cost-based fee" to copy your children's medical records. This cost is your responsibility. This fee may be waived if records are sent directly to our office.
If you have not already done so, please sign a release of records form and provide it to your children's previous medical provider.
Our Goal Is To Provide The Best Medical Care
To do this successfully, we must be able to review all of your medical records from all providers that you have seen. It is our policy to receive your previous medical records before we can assume care and schedule an appointment.
For Newborns
Bringing a newborn to a new pediatric practice can be a prudent decision to ensure comprehensive care and support for your child's lifelong health. It allows for establishing a rapport with the healthcare providers early on, fostering a sense of familiarity and trust crucial for ongoing care.
For New Families Transferring To Us
When transferring a child to a new pediatric practice, it's essential to gather and organize all medical records, including vaccination records and any relevant test results. Communicate with both the current and new pediatric practices to ensure a smooth transition of care, providing necessary information about the child's medical history and ongoing treatments.
